SUPPORTIVE CARE PROGRAMS AT STEDMAN COMMUNITY HOSPICE

Support During Serious Illness – Educational Series
Monthly educational support group providing a supportive, welcoming and inclusive space for individuals who are caregivers for someone with a serious illness or life-limiting diagnosis to connect with others. It provides a supportive, welcoming, and inclusive space to explore topics such as emotions, caregiver burnout, financial options and decisions, Advance Care Planning, coping with special occasions/gatherings, exploring grief and loss, self-care and support systems, etc. The group capacity will be held at 15 registered participants in person and 10 participants virtually. This group is facilitated by a member of the supportive care team.
Facilitated through a hybrid model (simultaneously - in person at Stedman Hospice and virtually) once per month on Thursday afternoon from 1:30 p.m. to 3:30 p.m.

Care for the Caregiver Support Group
Monthly support group for those who are caregivers for an individual with a palliative diagnosis providing a supportive, welcoming and inclusive space to connect with other caregivers experiencing similar challenges and experiences. Providing the opportunity to share, receive support and take part in a therapeutic activity, offering care for the caregiver. The group capacity will be held at 15 registered participants at Stedman Community Hospice. This group is facilitated by a member of the supportive care team.
The group will be held once per month on Thursday afternoon from 1:30 p.m. to 3:30 p.m.

CLOSED GROUP PROGRAMS
'Mixed Loss' Support Group
Mixed Loss Support Group is an 8-week grief support group open to individuals who have experienced the death of a significant individual in their life. This group aims to provide a supportive, welcoming and inclusive space for weekly support for any type of loss or grief. Participants eligible for this group will have experienced their loss from as early 6 weeks to 2 years prior to the first session. This group is open to adults who are seeking validation, normalization of experience and coping strategies as one learns to live with their grief. This group is delivered as a closed group, meaning the same participants make the commitment to meet weekly for 8 weeks. The program is held during the Winter, Spring and Fall of each year. The group capacity will be held at 10 registered participants at Stedman Community Hospice. This group is facilitated by a member of the supportive care team.
Winter Session: Tuesday February 10, 2026 to March 31, 2026 from 1:30 p.m. to 3:30 p.m.
Spring Session: Tuesday May 5, 2026 to June 23, 2026 from 1:30 to 3:30 p.m.
Fall Session: Tuesday, October 6, 2026 to November 24, 2026 from 1:30 p.m. to 3:30 p.m.

'Young Adults Mixed Loss' Support Group
The Young Adults Mixed Loss Support Group is an 8-week grief support group open to individuals between the ages of 20 – 35, who have experienced the death of a significant individual in their life. This group aims to provide a supportive, welcoming and inclusive space for weekly support for any type of loss or grief. Participants eligible for this group will have experienced their loss from as early 6 weeks to 2 years prior to the first session. This group is open to young adults who are seeking validation, normalization of experience and coping strategies as one learns to live with their grief. This group is delivered as a closed group, meaning the same participants make the commitment to meet weekly for 8 weeks. The program is held twice per year. The group capacity will be held at 10 registered participants at Stedman Community Hospice. This group is facilitated by a member of the supportive care team.
Spring Session: Thursday, May 7, 2026 to June 25, 2026 from 7 p.m. to 9 p.m.
Fall Session: Thursday, October 8, 2026 to November 26, 2026 from 7 p.m. to 9 p.m.

Partner/Spousal Loss Support Group is a 10-week grief support group open to individuals who have experienced the death of a partner or spouse. Providing a welcoming and inclusive space for weekly support for those facing the unique challenges of a partner/spousal loss from 6 weeks to 2 years post-loss. This group is open to those seeking validation, normalization of experience and coping strategies as one learns to live with their grief. This group is delivered as a closed group meaning, the same participants make the commitment to meet weekly for 10 weeks. The program is held during the Winter, Spring and Fall of each year. The group capacity will be held at 10 registered participants at Stedman Community Hospice. This group is facilitated by a member of the supportive care team.
Winter Session: Thursday January 22, 2026 to March 26, 2026 from 10:00 a.m. to 12:00 p.m.
Spring Session: Tuesday April 21, 2026 to June 23, 2026 from 10:00 a.m. to 12:00 p.m.
Fall Session: Thursday September 24, 2026 to November 26, 2026 from 10:00 a.m. to 12:00 p.m.

For individuals two months post-loss and offered for up to 1-2 years of participation. The group capacity will be held at 15 registered participants per gathering at Stedman Community Hospice. This group is appropriate for individuals who have previously taken part in a grief support group would like the opportunity for ongoing social connections. This gathering is supported by trained volunteers.
The gatherings are held once per month on Wednesday from 11:00 a.m. to 12:00 p.m.

The Canadian Mental Health Association (CMHA) Survivors of Suicide Loss Bereavement Support group is a support group for adults who have lost a family member or friend to suicide. This group provides a supportive, welcoming and inclusive space for anyone grieving the loss of a family member or friend by suicide.
Facilitated through a hybrid model (run simultaneously - in person at Stedman Hospice and virtually over Zoom) once per month on Wednesday evening from 7:00 p.m. to 9:00 p.m. **Note: No session held in August.

Brantford Circle of Support - Pregnancy and Infant Loss Support Group
PAIL Network offers peer-led in person support to all families across Ontario who have experienced a pregnancy loss, an infant death or elective abortion. Our team of trained volunteers provide support during monthly scheduled meetings. These groups connect families who share a similar experience of loss, and these discussions are moderated by a peer support volunteer who also shares that experience.
In-person monthly support sessions are held at Stedman Community Hospice once per month on Thursday evening of each month from 7:00 p.m. to 8:00 p.m., starting March 20, 2025. Registration is required to attend.
