How to Access Hospice Care
We know that it is often a very difficult and emotional decision for a patient or family member to know when to contact the Hospice for care and support. We want everyone to know that we are here for them, regardless of age, illness, race or economic status. In fact, it is our hope that people contact the Hospice from the very moment of diagnosis of a life-limiting illness, even if they may have many years to live. There is so much support we can offer along their journey as we are much more than a place that only provides care at end-of-life. Contact the Hospice directly for more program details at 519-751-7096 x2500, or search this site to learn more.
