Outreach Supportive Care
Stedman Community Hospice provides the community with a quick response team that is staffed with Palliative Care Physicians, Dr. Andrew Trevor, Dr. Bernadette McNeil, and Dr. Julie Gladstone; Clinical Nurse Specialists Nancy Morbrook and Shelley Groombridge; Supportive Care/Bereavement Advisors, Camillia Galezowski and Frank Manno and Administrative Assistant, Lynn Haig.

Whether you or a loved one has just been diagnosed with a life limiting disease, dealing with end of life issues or trying to cope with the loss of a loved one, our team is here to help. We also work closely with other community partners to assist you with your needs in the comfort of your own home.

Outreach Supportive Care Program can assist you with:
  • A consultation with one of the Hospice Palliative Physicians can be arranged in partnership with your family doctor to discuss pain and symptom issues.
  • Individual needs assessment for supportive, spiritual and advanced care as well as end of life planning.
  • One on one consultation with Supportive Care Coordinator to meet spiritual and psycho-social needs.
  • Grief and bereavement support for individuals coping with a diagnosis of a life limiting illness as well as support of those coping with the loss of a loved one.
